Comments:
An unannounced renewal study was conducted on 3/2/2020. At the time of entrance eight residents were in care. The sample size consisted of four resident records and two staff records. . Resident and staff records and other documentation reviewed. No new staff have been hired since last inspection. Residents were observed eating breakfast and engaging in activities including having a musician come and play the accordion and sing with residents. Medication administration was observed and exit interview held.

Violations:
Standard #: 22VAC40-73-650-A
Description: Based on documentation review and interview the facility failed to ensure, no medication, dietary supplement, diet, medical procedure, or treatment shall be started, changed, or discontinued by the facility without a valid order from a physician or other prescriber. Medications include prescription, over-the-counter, and sample medications.

As evidenced by:
Resident #3's most recent doctor's order dated 11/27/2019 and the MAR stated the resident was to receive Vitamin B12 100mcg. However, the medication the receiving was Vitamin B12 1,000mcg.

Plan of Correction: All prior doctor's orders had been for Vitamin B12 1,000 mcg which is the standard dosage for this medication. Immediately contacted the doctor to clarify the order and the order was verbally changed back to the intended B12 1,000 mcg. Paperwork and MAR were updated to reflect the change.
