Comments:

Two licensing inspectors conducted a one day unannounced complaint inspection at Commonwealth Senior Living in Radford on 03/11/2020. The inspection was in response to a complaint that was received by the licensing office on 02/18/2020. The inspection began at 11:00 am and concluded at 11:30 am. Resident records were reviewed and staff were interviewed related to allegations of one resident not receiving proper hygiene care. The preponderance of the evidence gathered and reviewed during the investigation does not support the allegations, so the complaint is determined to be not valid. An exit meeting was held with the administrator by telephone and other key staff on site at the facility on 03/11/2020 and at that time the findings of the investigation were shared.
