Comments:
Two licensing inspectors conducted a focused monitoring visit regarding a self reported incident at Commonwealth Senior Living In Radford on 03/11/2020. The monitoring inspection began at 9:50 am and concluded at 11:00 am. The licensing inspectors observed documentation to include Medication Administration Records, physician's orders, nurses's notes, physician's notes, and other reports. Staff interviews were also conducted. An exit meeting was held with key staff and the administrator was included by telephone, as she was not in the facility on this date. One violation is being cited as a result of this monitoring inspection. Please complete the "description of action to be taken" and "date to be corrected" for each violation cited on the violation notice, and then return a signed and dated copy to this licensing office within 10 calendar days (03/27/2020) of receipt. and If you have any questions or concerns please do not hesitate to contact your inspector at 276-608-1067. Standard #: 22VAC40-73-680-D
Description: Based on interviews with staff, the facility failed to administer medications in accordance with the physician's or other prescriber's instructions and consistent with the standards of practice outlined in the current registered medication aide curriculum approved by the Virginia Board of Nursing
EVIDENCE:
1. Resident # 1 is prescribed the following medications for January 2020 to be administered at 6:30 am: Esomeprazole mag cap 20mg, take one capsule by mouth every day. The following medications are to be administered at 7:30 am for January 2020: Buspirone 5mg one tablet twice daily; Celecoxib 200 mg take one capsule by mouth daily; Duloxetine 60mg take one capsule by mouth daily; Eliquis 5mg take one tablet by mouth twice daily; Ferrous Sulfate 325mg tale two tablets by mouth three times weekly on Monday, Wednesday and Friday; Fexofenadine 180 mg one tablet by mouth daily; Fluticasone prop 50mcg, spray one spray into each nostril every day; one spray into each nostril every day; Linzess 145mcg one capsule by mouth every day before a meal; Movantik 25 mg one tablet by mouth every day; Symbicort 160-4.5 mdg inhale one puff by mouth twice daily; Hydrocortisone 1% cream apply to affected area topically twice daily; Prednisone 10mg take one and a half tablet daily for 30 days.
2. On a telephone call on 03/16/2020 Staff #1 stated that she was supposed to administer Resident #1's medications at 7:30 am but did not administer them to her until noon. Staff #1 could not give a specific date of this incident, but was certain it was an isolated incident and happened in the later part of January 2020.

Plan of Correction: A complete investigation was completed by the Executive Director to include reviewing MAR, resident interview, camera review, and associate interviews on 01/27/2020. Resident received morning medications around noon that day. Proper personnel corrective actions were taken. The associate as of 1/31/2020 is no longer employed by the company. Cart audit was completed by Resident Care Director and all counts and medications were correct. [sic]
