Comments:
An unannounced complaint inspection was conducted by two Licensing Representatives on March 9, 2020 from 10:31 a.m. to 12:00 p.m. in response to allegations of short housekeeping and direct care staffing and housekeeping concerns in the Special Care Unit (SCU). There were 53 residents in care. The Executive Director was present throughout the inspection. A tour was conducted of the SCU during the inspection. Resident rooms and restrooms were inspected and staffing schedules were reviewed. The following topics were discussed: documenting meetings with residents/responsible parties, documenting schedule changes on housekeeping schedule. The complaint is not valid.

Please submit your ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it within 10 calendar days. The plan of correction must indicate how the violation will be or has been corrected. The plan of correction must include: 1. Step(s) to correct the noncompliance with the standard(s) 2. Measures to prevent reoccurrence 3. Person(s) responsible for implementing each step and/or monitoring any preventative measures.

Description: Based on observation and interview, the facility failed to store cleaning supplies and other hazardous materials in a locked area.

Evidence:

1. In the Special Care Unit (SCU) near room H105, a housekeeping cart was observed unsupervised and unattended at approximately 10:46 a.m.

2. Staff #2 assigned to the cart was down the hall from the cart, out of sight, when the cart was observed.

3. The housekeeping cart contained gunk and adhesive remover gel, all purpose spot and stain remover, and peroxide multi-surface cleaner and disinfectant. The items were in the top of the cart in the unlocked portion of the cart.

4. Additionally, a clear bottle labeled ?Fabuloso? was observed in the doorway on the floor of room H105 across from the cart.

5. When asked by staff #1 why the cart was unlocked, staff #2 stated, ?It won?t lock.?

6. Staff #1 and staff #2 observed the unsupervised housekeeping cart, and the cleaning supplies and other hazardous materials were not stored in a locked area.

Plan of Correction: Maintenance Director repaired lock on cart at time of inspection.

Maintenance Director provided in-service to housekeeping staff on daily cart inspections to include locks and requirements of standard for the storage of cleaning supplies and other hazardous materials.

Maintenance Director/Designee will monitor housekeeping carts for compliance.
