Technical Assistance:

It is recommended that a record be maintained either on the medication administration record (MAR) or in another form to indicate how many units of insulin is being given when a sliding scale is in place. The MAR is the most reasonable place to have it and is easier to monitor.

Comments:

This inspection was conducted by licensing staff using an alternative remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 5/29/2020 and concluded on 6/3/2020. The administrator was contacted via email to initiate the inspection. The administrator reported that the current census was 15. The inspector emailed the administrator the list of items required to complete the inspection. The inspector reviewed 2 resident records plus one additional resident medical record, 2 staff records plus training, background checks for all new employees since last inspection, outside inspections and related drills, and staff schedules submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or the law in one area, and a violation was documented on the violation notice issued to the facility. Standard #:	22VAC40-73-680-D
Description:	Based on a desk review of medication administration records and physician orders, resident C received insulin outside the parameters of the physician ordered sliding scale. What was received could not be determined as the medication record does not currently have a place to indicates units given.
Plan of Correction:	Staff administering medication will be provided additional training regarding following a sliding scale. Resident C is returning home by his own choice. The administrator will ensure correction and future compliance.
