Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. Allegations (three separate reports) were received anonymously through the Department's on-line complaint system regarding staffing, facility cleanliness, medication administration, staff to resident interactions, nutrition and weight loss, hydration, etc. and investigated on 6/28/2020. Based on documentation reviewed and interviews with facility staff, the allegations were determined to be not valid. Note: The licensing inspector attempted to contact a call back number left on one of the anonymous complaints, the calls were discontinued by someone hanging up the phone with no response. Several calls were made to the licensing inspector's telephone on the same day. When the telephone was answered by the licensing inspector, the individual hung up the telephone and discontinued the telephone call.
