Comments:
A Representative with the Division of Licensing, conducted an unannounced, mandated, monitoring inspection on 02/28/2020 from approximately 10:52am to 3:33pm. At the point of entrance the facility Administrator was not available however staff in charge were available to assist with the inspection. The Licensing inspector observed the physical plant, reviewed 10 residents records and 5 staff records, observed residents during meals and activities, and observed the facility medication administration pass. Areas on non-compliance are found within this violation notice. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return to me within 10 calendar days from today. Your plan of correction must contain: 1. Steps to correct the noncompliance with the standard(s) 2. Measures to prevent the noncompliance from occurring again 3. Description: Based on observation of the facility physical plant, the facility failed to ensure when there are indicators that ordinary materials or objects maybe harmful to a resident, these material or objects shall be inaccessible to the resident except under staff supervision.


Evidence: On 02/28/2020 on the facility memory care unit, in front of resident room #209 with staff #1, a flat rolling cart with hazardous work tools such as hammer, screw drivers, wrench, two, five gallon buckets of paint, and one, one gallon of paint was left unattended and not locked with two residents sitting close by ( as evidenced by photo provided).
