Comments:

This inspection was conducted by licensing staff using an alternative remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated May 27, 2020 and concluded on June 22, 2020. The Administrator was contacted by telephone to initiate the inspection. The Administrator reported that the current census was 64. The inspector emailed the Administrator a list of items required to complete the inspection. The inspector reviewed 4 resident records, 4 staff records, staff scheduled for the past 2 weeks, mot recent Health Care Oversight, most recent Fire Inspection Report, most recent Health Department Inspection Report, Fire and Emergency drills for the past 3 months, most recent Oversight by dietitian or nutritionist, staff Criminal Record Reports and Sworn Disclosure Statements for new hires since the last renewal inspection. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issues to the facility.

Description:	Based on record review and discussion, the facility failed to ensure the physical examination report (report) listed the description of the person?s reactions to known allergies. Evidence: Resident #1?s physical examination report dated March 2, 2020 did not document the description of reactions to known Vioxx allergies.
Plan of Correction:	1 Directed by Physician to write in Resident's reaction to Vioxx on Resident's original History & Physical which was cited. Prior to being admitted, prospects History & Physicals will be reviewed for accuracy in the "Allergies section" and initialed.
