Description:	Based on interview, facility failed to ensure that the facility shall immediately notify the Virginia Board of Long-Term Care Administrators that the licensed administrator resigned and that a new licensed administrator has been employed or that the facility is operating without an administrator licensed by the Virginia Board of Long-Term Administrators, whichever is the case, and provide the last date of employment of the previous licensed administrator. Evidence: Department's regional licensing office was notified on 5/15/2020 of the start of the new acting administrator and was notified on 7/2/2020 that the Virginia Board of Long-Term Care Administrators had not been notified to date.
Plan of Correction:	The Board of Long -Term Care Administrators was notified on 7/2/2020 that a temporary Administrator had been hired and a new Administrator will assume the position on 8/3/20. Senior Executive Director, Amber Ralls will be responsible for monitoring changes and notifying the board of any future change in Administration.

Description:	Based on interview, facility failed to ensure that when an acting administrator is named and is intending to assume the position permanently, he shall submit a completed application for an approved administrator-in-training program to the Virginia Board of Long-Term Care Administrators within 10 days of employment. Evidence: Department's regional licensing office was notified on 5/15/2020 of the start of the new acting administrator and was notified on 7/2/2020 that the application for an approved administrator-in-training program had not been submitted to date.
Plan of Correction:	Application for an approved administrator-in -training program was submitted to the Board of Healthcare Professionals on 7/2/20. Future compliance will be monitored by the Senior Executive Director, Amber Ralls and the facility administrator. Amber Ralls will be responsible for precepting and overseeing the AIT program. Any changes will be communicated with the board and the licensing inspector.
