Subjectivity

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 07/29/2020 and concluded on 07/30/2020. A self-reported incident was received by the department regarding allegations in the areas of 1090 A.. The Administrator was contacted by email to conduct the investigation. The licensing inspector emailed the Administrator a list of documentation required to complete the investigation.

The evidence gathered during the investigation supported the self-report of non-compliance with standards or law, and violations were issued. Any violations not related to the self-report but identified during the course of the investigation can be found on the violation notice.

Violations:
Standard #: 22VAC40-73-1090-A
Description: Based on information received as part of a facility self-reported incident, the facility failed to ensure an assessment was completed by an independent physician determining one resident had a serious cognitive impairment due to a primary psychiatric diagnosis of dementia prior the placement of the resident in a safe, secure environment.

EVIDENCE:
1. Resident # 1 was placed in the safe secure unit provided by the facility upon her exiting the building from the Assisted Living part of the facility on 07/28/2020.
2. The Assessment for a safe secure unit signed by the nurse practitioner was completed on 07/29/2020 which was after the placement of resident # 1 in the safe. secure unit.

Plan of Correction: 1. Signed verbal order will be obtained from physician for emergency transfers to SCU until Serious Cognitive Assessment form is completed.
2. Facility will provide increased supervision to any future resident that exhibit this behavior for their safety. [sic]
