Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A complaint inspection was initiated on 06/03/2020 and concluded on 07/09/2020. A complaint was received by the department regarding allegations in the areas of 22VAC40-73-(6) RESIDENT CARE AND RELATED SERVICES. The Administrator was contacted by telephone to conduct the investigation. The Licensing Inspector emailed the Administrator a list of documentation required to complete the investigation. The evidence gathered during the investigation supported the allegation of non-compliance with standards or law, and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Description: Based on record review the facility failed to ensure at the time the medication is administered, the facility shall document on a medication administration record (MAR) all medications administered to residents, including over-the-counter medications and dietary supplements.

Evidence #1: On 07/09/2020 while reviewing resident record #1, it was observed that Hyrdocodone-acet 5mg -325mg tablet to be administered oral three times a day as needed was not signed for on the medication administration record at the time of administration on 04/11/2020, 04/15/2020, 04/16/2020, 04/17/2020, 04/20/2020 and 04/21/2020.

Evidence #2: On 07/09/2020 while interviewing staff #3, staff #3 stated, "the medication was administered to the resident and documented on the narcotic sheet". Staff #3 confirmed that documentation was not made on the medication administration record at the time of administration.

Evidence #3: On 06/03/2020 while interviewing staff #2 regarding medication non-compliance, staff #2 informed the Licensing Inspector that staff #3 and #4 did not document on the facility medication administration record for medications administered to resident #1 at the time of administration. Staff #2 informed the Licensing Inspector that the non-compliance had been addressed with staff #3 and #4.
