Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A complaint inspection was initiated on 8/6/2020 and concluded on 8/14/2020. A complaint was received by the department regarding allegations in the areas of discharge of residents. The administrator was contacted by telephone to conduct the investigation. The licensing inspector emailed the administrator a list of documentation required to complete the investigation.

The evidence gathered during the investigation supported the allegation of non-compliance with standards or law, and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Based on document review and interview, the facility failed to ensure that any resident of an assisted living facility has the rights and responsibilities as provided in 63.2-1808 of the Code of Virginia, specifically by being afforded reasonable assistance to ensure an orderly transfer or discharge.

EVIDENCE:

1. Resident 1 was discharged from the facility on 7/17/2020; however, at the time of departure, the facility failed to provide all of the resident?s diabetic syringes. Per interview with staff 1, resident 1 is required to take three insulin injections per day; however, staff 2, who was the RMA on duty on the day of departure, withheld all but five of resident 1?s syringes upon discharge
2. Per collateral interview, additional syringes had to be obtained out of pocket in order to ensure that the resident had continuity of his diabetic medical supplies.

Plan of Correction: Administrator to review facility discharge policy with staff 2, RMA on duty. At the time of discharge, facility is to provide all medical supplies in addition to medications to the resident or resident representative. If safety concerns are present all supplies are to be provided followed with an APS referral for wellness check.
