Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initialed on 08/26/2020 and conducted on 09/01/2020. The administrator was contacted by telephone to initiate the inspection. The administrator reported that the current census was 16. The inspection emailed the administrator a list of items required to complete the inspection. The inspector reviewed two resident records, two staff records, facility schedule, fire inspection, health inspection, diet and medication reviews submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards of law, and one violation was documented on the violation notice issued to the facility. A corrective action plan should be for each violation cited along with a date of correction and return a singed and dated copy back to the licensing office within 10 calendar days (09/11/2020) of receipt. Description:	Based on documentation review, the facility failed to maintain a written work scheduled that indicates who is in charge at any given time. EVIDENCE: 1. On the schedules dated for August 9-15, 2020 and August 16-22, 2020 there was no way to determine who was in charge at any given time at the facility.
Plan of Correction:	Time sheet has "DSP" added, which is "Designated staff person in charge". This correction will be on all time sheets in the future. [sic]
