Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An initial inspection was initiated on 09-02-2020 and concluded on 09-02-2020. The Administrator was contacted by telephone to initiate the inspection. The inspector reviewed areas related to buildings and grounds, resident accommodations and related provisions, emergency preparedness, and additional requirements for facilities that care for adults with serious cognitive impairments; to include measurements, call bells, door and window locks on the special care unit, furniture used by residents, emergency food and water supply, generator, first aid kits, water temperatures, and fire and evacuation drawings.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-73-980-B
Description: Based on observation and interview, the facility failed to ensure in facilities that have a motor vehicle that is used for a field trip and is used to transport residents, there should be a first aid kit on the vehicle that includes the required items; to include plastic bags, tweezers, thermometer, and a blanket.
Evidence:
1. During a virtual review of the facility?s first aid kit located on the facility?s van with staff #1 and staff #2, the kit not include plastic bags, tweezers, thermometer, or a blanket.
2. Staff #1 stated that the van will be used by the facility to transport residents. When asked if the facility?s van first aid kit contained the aforementioned required items, staff #1 stated ?Not in this one.?

Plan of Correction: Plastic bags, tweezers, thermometer, and blanket has been ordered for the first aid kit located on the van. The Activities Director will check-off and initial the first aid kit sheet monthly to ensure all items are there. The Executive Director will do random audits to ensure all first aid kits are within compliance with DSS standards.
