Comments:
This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 7-28-20 and concluded on 8-18-20. A complaint was received by the department regarding allegations in the area of personal care. The administrator was contacted by telephone to conduct the investigation. The licensing inspector emailed the administrator a list documentation required to complete the investigation.
The evidence gathered during the investigation did not support non-compliance with standards or law, and violations were issued. Any violations not related to the complain but identified during the course of the investigation can be found on the violation notice.

Description: Based on document review and staff interview, the facility failed to ensure the resident?s individualizes service plan was updated as needed as the condition of the resident changes. The update shall be performed by a staff person with the qualifications specified in the regulations and in conjunction with the resident and, as appropriate, with case manager and health care providers.

Evidence:
1. During a remote complaint inspection, a review of resident #1?s nurse?s notes dated 8-4-20 documented resident receiving physical therapy.
2. Further review of resident #1?s physical therapy notes received on 8-17-20, the resident receives outpatient physical therapy at a local facility. The physical therapy schedule (June- July August calendar) document resident #1?s physical therapy started June 25, 2020 and the last appointment on the calendar is August 20, 2020. Therapy is scheduled for at least twice a week.
3. During interview with staff #1 on 8-14-20, resident #1 was assessed for nursing home placement. The facility?s license is for residential level of care only. Staff #1 also acknowledged resident #1 was receiving therapy services.

Plan of Correction: All client status to be monitored for changes weekly and ISP updated accordingly by Administrator and/or designee.
