This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A complaint investigation was initiated on 11/16/2020 and concluded on 1/12/2021. The Administrator was contacted by phone to initiate the complaint investigation. The inspector reviewed one resident record and conducted interviews with family member, case manager and facility staff. Information gathered during the inspection determined non-compliance(s) with applicable standards or law. Violations were documented on the violation notice issued to the facility. Standard #: 22VAC40-73-70-A Complaint related: Yes Description: Based on review of Resident #1's record and an interview with Staff #1, the facility failed to ensure that an incident that negatively affected or that threatened the life, health, safety, or welfare of any resident was reported to the regional licensing office within 24 hours.
Evidence: The Licensing Inspector (LI) did not receive notification of an incident that occurred on 11/12/2020 until 11/16/2020 after the LI reached out to the Staff #1 for information regarding the incident. During an interview with LI, Staff #1 confirmed that an incident report was not submitted within 24 hours.Plan of Correction: Administrator/Owner will ensure that all incidents are reported to the licensing office in a timely manner.
