Comments:

This inspection was conducted by Licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 1/20/2021 and concluded on 1/25/2021. The Manager was contacted by telephone to initiate the inspection. The Manager reported that the current census was 4. The Inspector emailed the Administrator and Manager a list of items required to complete the inspection. The Inspector reviewed 2 resident records, 2 staff records, physician's orders, medication administration records, fire and health inspections, health care oversight, fire drills, staff schedules submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non compliance with applicable standard or law and violations were documented on the violation notice issued to the facility. Standard #:	22VAC40-73-450-A
Description:	Based on a review of resident's individualized service plans, one out of two plans reviewed did not have all resident's needs addressed. Evidence: Resident's #1 physical documents there is some difficulty in transferring and may require additional staff to assist with transferring which is not addressed on the preliminary or comprehensive ISP.
Plan of Correction:	Administrator and designated staff will review all resident's physical forms make sure to include needs to be addressed on the individualized service plans. Needs omitted will be added to resident 's #1 plan to address and monitor for any changes.
