Technical Assistance:
An addendum will be provided to each resident's agreement and a new facility disclosure statement will reviewed and signed by the resident or legal representative. Criminal history reports will be obtained on all staff and a new sworn statement of affirmation for adult facility employees will be signed and maintained on file in the facility.

Comments:
An announced remote initial inspection was conducted from 02/01/2021 through 02/02/2021 in response to a new application for licensure related to a change of licensee. There are currently 46 residents in care. Policies and procedures are being submitted for review and discussed with the administrator to ensure requirements are met per Commonwealth of Virginia State Board of Social Services Standards for Licensed Assisted Living Facilities 22 VAC 40-73. All required inspections are current as per the governor's guidance related to the pandemic.. Written notification of change in licensee was provided to residents and/or legal representatives on 11/01/20 in anticipation of the February change over. The new licensee will utilize the Virginia Department of Social Services model forms. A monitoring inspection will be completed in sixty days to ensure compliance.
