Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A complaint inspection was initiated on 03/02/2021 and concluded on 03/03/2021. A complaint was received by the department regarding allegations in the areas of resident care. The director of nursing was contacted by telephone to conduct the investigation. The licensing inspector emailed the director of nursing a list of documentation required to complete the investigation.

The evidence gathered during the investigation supported the allegations of non-compliance with standards or law, and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Description: Based on review of the resident record and the medication administration record (MAR), the facility failed to ensure methods were being followed to ensure that prescription medications and any over- the- counter drugs and supplements ordered for the resident are filled and re-filled in a timely manner to avoid missed doses for one resident in care.

EVIDENCE:
1. Resident # 1 is prescribed Acetamiophen-codeine 300-30mg for pain take one tablet by mouth at bedtime. According to documentation on the MAR this medication was unavailable for administration to resident # 1 on 02/1, 11, 12, 13, 14, 16, 23, 24, 25,26, 27, 28, & 03/01/2021.
2. Resident # 1 is prescribed Lactulose 20 gram/30 mL for increased ammonia levels to administer 30 cc orally once a day. According to documentation on the MAR this medication was unavailable for administration to resident # 1 on 02/01/2021.
3. Resident # 1 is prescribed Latanoprost drops 0.005% to administer one drop in each eye daily at bedtime for glaucoma. According to documentation on the MAR this medication was unavailable for administration to resident # 1 on 02/12, 13, &14, 2021.

Plan of Correction: Resident # 1: Acetamiophen-codeine 300-30mg, Lactulose, and Latanoprost was available on medication cart on 03/02/2021. Medication availability and available on the cart for other residents completed by RCDNS/Designee by (03/19/2021). Follow up based on findings. [sic]

RMA's educated by RCDNS/Designee that medication ordered shall be on medication cart. Medications will be assessed daily by RMA's to ensure reordering when meidcation is down to a 5 day supply. RCDNS/Designee will monitor weekly times 12 weeks. [sic]
