Comments:
This complaint inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint was filed with the licensing office indicating the facility failed to report an incident in a timely manner. An additional complaint was received at the same time alleging neglect related to resident care including misuse of medication. The first part of the complaint was found to be valid and a violation issued. The second part of the complaint could not be substantiated based on the information that could be obtained from the facility. No additional information was provided by the complainant despite multiple efforts to contact.

Complaint related: No
Description: The facility failed to report a major incident involving resident A within 24 hours of the occurrence. The incident required that the resident be sent to the emergency room. The family further indicates they were notified by the hospital and not the facility.

Plan of Correction: All incidents will be reported within 24 hours of occurrence either by email or fax. It is understood that the full report is due in the licensing office within seven days of the occurrence. The facility nurse and or administrator assume responsibility. In the absence of access to faxing or email it is understood a message can be left on the phone of the licensing inspector at (540) 292-5930. Families will be notified at the time of the incident and documented as such even if a message has to be left. Our facility tries to have a secondary contact but that is not always possible.
