Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A complaint inspection was initiated on March 18, 2021 and concluded on March 24, 2021. A complaint was received by the department regarding allegations in the areas of resident care, staffing, and buildings and grounds. The executive director was contacted by telephone and a virtual complaint inspection was conducted during the investigation. The licensing inspector also emailed the executive director a list of documentation required to complete the investigation. The evidence gathered during the investigation did not support all of the allegations of non-compliance with standards or law. However, a violation was issued in the area of resident care for one resident and can be found on the violation notice.

Description:	Based on a review of facility documentation, the facility failed to ensure that personal assistance and care was provided to each resident as necessary with bathing at least twice a week, but more often as needed or desired. Evidence: Facility documentation indicated that Resident # 5 who resides on the Assisted Living unit, only received a shower on March 3, March 13, and March 18, 2021.
Plan of Correction:	Resident number 5 will receive a bath or shower at least 2 times per week and will be documented on the ADL sheet that this task has been completed. All resident care staff will complete a documentation Inservice to ensure that an understanding of what is to be documented and how tasks are to be documented is completed no later than April 20, 2021.
