Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 4/13/2021 and concluded on 4/13/2021. A self-reported incident complaint was received by the department regarding allegations in the area of personnel. The licensee contacted the LI by text. The existing texts and phone conversation were sufficient to complete the inspection. The evidence gathered during the investigation supported the allegation of non-compliance with standards or law, and violations were issued. Any violations not related to the monitoring inspection but identified during the course of the investigation can be found on the violation notice.

Description:	Based on written communication and phone interview, the facility failed to have an administrator of record. EVIDENCE: 1. On 3/12/2021 the administrator stated in a phone interview that the facility no longer had an administrator. 2. On 3/15/2021 one of the licensees sent a text stating the administrator quit on Friday (3/12/2021). A text later in the day from the same person stated she would not become the administrator again.
