Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 04/07/2021 and concluded on 04/22/2021. A self-reported incident was received by the department regarding allegations in the areas of resident care. The administrator was contacted by telephone to conduct the investigation. The licensing inspector emailed the administrator a list of documentation required to complete the investigation.

The evidence gathered during the investigation supported the self-report of non-compliance with standards or law, and violations were issued. Any violations not related to the self-report but identified during the course of the investigation can be found on the violation notice.

Violations:
Standard #: 22VAC40-73-660-A-2
Description: According to documentation received as part of self-reported incident, the facility failed to ensure Schedule II drugs and any other drugs subject to abuse must be kept in a separate locked storage compartment within a cabinet or cart.

EVIDENCE:
1. According to documentation review and email correspondence with the administrator on April 4, 2021 the administrator received a call that there were some narcotics in a medicine cup that had been stapled shut in the top drawer of medication cart A.
2. Upon further investigation it was determined the narcotics in the medication cup belonged to residents within the facility and no medications appeared to be missing. The Narcotics found in the top drawer of Medication Cart A were from different medication carts.
3. According to the Medication Administration records all residents had received their schedule II medications.
4. The top drawer of Medication Cart A where the narcotics were found was not a double locked container within the medication cart.

Plan of Correction: All narcotics will be kept in a locked container within the med cart, so all narcotics will be double locked.

RMA's re-educated on storage of medications to include narcotics. Administrator will monitor to assure proper storage of medications. [sic]
