Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A complaint inspection was initiated on 2/27/2021 and concluded on 5/5/2021. A complaint was received by the department regarding allegations in the areas of resident care and related services, and buildings and grounds. The administrator was contacted by telephone to conduct the investigation. The licensing inspector emailed the administrator a list of documentation required to complete the investigation. The evidence gathered during the investigation supported one of the allegations of non-compliance with standards or law, and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Description:	Based on interview, the facility failed to keep furniture clean and in good condition. 1. The mattress used by residents 1 and 2 was soiled and had a bad odor. This was confirmed by staff 1, who had the mattress replaced prior to this investigation.
Plan of Correction:	The mattress was replaced prior to inspection. From now on, direct care staff and housekeeping staff will be instructed to inform management in writing if a similar situation occurs
