Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A unannounced focused inspection was conducted to investigate allegations concerning credentials and training of administrator, visitation policy and cleanliness of the building. The administrator was contacted by telephone to initiate the inspection. The administrator reported that the current census was 62. The inspector emailed the administrator a list of items required to complete the inspection. The inspector reviewed the requested facility documents. There was no specific resident identified in the report to the Virginia Department of Social Services-Adult Licensing Division. Based on allegations reported, the following was reviewed as a part of the investigation: administrator license status, and training, policy regarding visitation during resident end of life, facility maintenance, invoices for cleaning services and upgrades. There was no preponderance of evidence found to substantiate allegations. The information gathered during the inspection determined no violations with applicable standards or law.
