Comments:

A non-mandated monitoring inspection was initiated on 07/16/2021 and concluded on 07/16/2021. A self-report from the facility was received by the department regarding the area of personnel. The Licensing Inspection spoke with the Community Director regarding the self-report. The evidence gathered during the non-mandated monitoring inspection supported the self-report of non-compliance with standards or law, and violations were issued. Any violations not related to the self-report but identified during the course of the investigation can be found on the violation notice.

Standard #:	22VAC40-73-150-A
Description:	Based on staff email and interview, the facility failed to ensure there is an administrator of record. EVIDENCE: 1. In an email from staff 1 on 07/14/2021 at 5:04 PM, staff 1 stated that the administrator of record, staff 2, was removed from payroll as of 06/30/2021. 2. Interview with staff 1 on 07/15/2021 confirmed that there has been no administrator of record at the facility since 06/30/2021.
Plan of Correction:	Staff 1 is in process of hiring a new AL Administrator of Record or attempting to get approval for a AIT for a current staff from Virginia Board of Long Term Care Administrators. Staff 2 was removed from payroll due to an unforeseen event.
Standard #:	22VAC40-73-150-B-2
Description:	Based on staff interview, the facility, licensed for both residential and assisted living care, failed to immediately notify the Virginia Board of Long-Term Care Administrators that the licensed administrator resigned or was discharged from the facility, is operating without an administrator licensed by the Virginia Board of Long-Term Administrators, and provide the last date of employment of the previous licensed administrator. EVIDENCE: 1. Interview with staff 1 on 07/15/2021 confirmed that the Virginia Board of Long-Term Care Administrators had not been notified that the facility?s administrator, staff 2, is no longer the facility?s administrator of record and was removed from payroll on 06/30/2021.
Plan of Correction:	Staff 1 notified Long Term Care on 7/15/2021 that Staff 2 was removed from payroll on 6/30/2021. Staff 1 misunderstood that Virginia Board of Long Term Administrators was required to be notified within a specified timeframe.
