Comments:

A renewal inspection was initiated on 8/16/2021 and concluded on 8/30/2021. The administrator was contacted by telephone to initiate the inspection. The administrator reported that the current census was 14. The inspector emailed the administrator a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed (2) two resident records, (2) two staff records, activities calendar, staff schedules, health care oversight, medication administration records etc.) submitted by the facility to ensure documentation was complete. The inspector conducted the on-site portion of the inspection on 8/30/2021. An exit interview was conducted with the Administrator 8/30/2021 on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Description:	Based on a review of resident records the facility admitted an individual to the facility before a determination could be made if they had any of the prohibited conditions or care needs listed in the standard. Evidence: Resident #2 was admitted to the facility on 6/2/2021. His admission physical exam did not contain an assessment of the conditions or care needs that could prohibit his admission to the facility.

Standard #:	22VAC40-73-320-A
Description:	Based on a review of resident records the facility failed to obtain the results of a risk assessment documenting the absence of tuberculosis for residents as evidenced by the completion of the current screening form published by the Virginia Department of Health. Evidence: Resident # 2 was admitted to the facility on 6/2/2021. A tuberculosis screening was not completed as a part of his admission physical examination.
