A non-mandated monitoring inspection was initiated on 9/21/2021 and concluded on 9/21/2021 to follow-up on compliance with an Intensive Plan of Correction (IPOC) issued August 2021 as a result of a high risk violation issued in April 2021. The licensing inspector conducted an on-site inspection of the facility on 9/21/2021 and observed the shift communication log book, sign out log and signaling system for compliance. The facility was found in compliance with the IPOC.
