Comments:

A complaint inspection was initiated on 08/18/2021 and concluded on 09/21/2021. A complaint was received by the department regarding allegations in the areas of staffing and building and grounds. The Administrator was contacted by telephone to conduct the investigation. The licensing inspector emailed the Administrator a list of documentation required to complete the investigation. The evidence gathered during the investigation supported the allegations of non-compliance with standards or law, and violations cited can be found on the violation notice.

Description:	Based on a review of facility employee schedules and staff interviews, the facility to ensure that the required number of direct care staff were on duty at all time in the special care unit. EVIDENCE: 1. The facility employee schedule has documentation that only 2 direct care members were scheduled to work on the special care unit on the 10:45pm to 7:15am shift on 08/06/2021 and 08/21/2021. 24 residents, including 1 resident who requires 2 person assistance with transfers was noted to be residing on the special care unit on these shifts, which would require a minimum of 3 direct care staff to be working on the unit.
Plan of Correction:	Staffing shall meet the requirements per DSS guidelines. Staffing schedules will be reviewed daily by scheduler and ARCD to ensure appropriate coverage. RCD to review weekly and ED will review schedules monthly to ensure compliance. Ed, RCD, ARCD and Scheduler.
