Comments:
A non-mandated complaint inspection was initiated on 10/28/2021 and concluded on 11/01/2021. A complaint was received by the department regarding allegations in the areas of admissions/discharge. The administrator was contacted by telephone to conduct the investigation. The licensing inspector emailed the administrator a list of documentation required to complete the investigation

The evidence gathered during the investigation supported the allegation(s) of non-compliance with standards or law, and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Complaint related: No
Description: Based on documentation review and an interview with the administrator, the facility failed to inform the legal guardian of an emergency discharge for one resident in care.

EVIDENCE:
1. According to documentation submitted by the facility and an interview with the administrator, resident # 1 had a mental health episode on October 20, 2021. This mental health episode resulted in resident # 1 to be voluntarily committed to a local psychiatric facility and being discharged from the facility.
2. The administrator stated there was paperwork for resident # 1 that he has a guardian. The guardianship agency was not contacted by the facility to inform them that resident # 1 was in an inpatient psychiatric facility and being discharged from the assisted living facility. The guardianship agency was contacted on October 26 by a case manager at the psychiatric facility informing them resident # 1 was there. The administrator of the assisted living facility stated "she forgot to contact the guardian for resident # 1".

Plan of Correction: When a resident has an incident at the facility, whether they are hospitalized ir nit, their contact person will be notified immediately or within 24 hours. The contact person will also be notified if the resident is to be discharged. [sic]
