Comments:

An unannounced monitoring inspection was completed at the facility on 11/19/2021 from 10:30 am to 12:30 pm. The administrator was on site during the inspection and provided requested documents. the administrator reported three residents in care, two resident files and two staff files were reviewed for compliance along with fire evacuation drills, required postings, health and fire inspection reports, menu, activity calendar, etc. The building was inspected and found clean and in good maintenance. The residents' rooms and shared spaces were clean and comfortably furnished. Residents expressed satisfaction with care and services provided. An exit interview was conducted with the administrator on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any additional information or documentation. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations are documented on the violation notice issued to the facility.

Description:	Based on a review of two resident files, the facility failed to provide a current disclosure statement developed by the department to the resident and his legal representative. Evidence: The disclosure statement in the residents' files is not the current form developed by the department dated 10/19 and did not include information regarding an on site electrical power source, disclosure of liability insurance or the initials of the resident and/or his legal representative.
