Comments:

The Licensing office received a complaint on November 9, 2021 regarding resident discharge at the Carrington Place of Wytheville facility. Based on information gathered during the investigation the compliant is found to be valid. After reviewing documentation during the inspection one violation is being cited. Please develop a plan of correction for the cited violation along with a date of correction and return a signed copy back to the licensing office within 10 calendar days (12/09/2021) of receipt. Description:	Based on documentation review, the facility failed to issue a refund within 60 days of the date of discharge for one resident. EVIDENCE: 1. Resident # 1 was discharged from the facility on February 10, 2021. According to correspondence with the facility and a review of resident # 1's final statement of accounts, he was owed a refund in the amount of 1,007.89. This refund amount was issued to resident # 1 on November 17, 2021 which exceeds 60 days from the discharge date.
Plan of Correction:	Resident # 1 was issued a refund in the amount of 1,007.89 from Birdmont Healthcare, LLC on11/17/21. Business office manager will audit the account of all discharged residents monthly as to comply with state regulation as stated "within 60 days of the date of discharge, each resident or his legal representative shall be given a final statement of account, any refunds due, and return of any money, property, or things of value held in trust or custody by the facility. [sic]
