Comments:
Two Licensing Inspectors conducted an unannounced non-mandated monitoring inspection on 12/07/2021, and additional documentation requested was received on 12/20/2021. The Licensing Inspection was in reference to the facility issued Provisional License. During the inspection the facility staff in charge was available and the facility Administrator arrived later to continue the inspection. The Licensing Inspectors observed the facility physical plan and interviewed facility residents and staff.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-73-100-A
Description: Based on resident interview and review of the facility infection control policy, the facility failed to ensure the assisted living facility shall implement the infection control program. Evidenced by photos

On 12/07/2021 While observing the facility upstairs restroom, with two Licensing Inspectors and staff #2, resident # 2 was observed going into the restroom (in which the bathroom sink was located in the bathroom shower), with a roll of toilet tissue. Resident #2 returned from the restroom and the Licensing Inspector asked resident #2 if the resident was able to wash hands. Resident #2 informed the Licensing Inspector that the bathroom did not have a sink to wash the resident hands. Staff #2 did not direct resident #2 to any additional areas of hand washing nor offer sanitizer.

Evidence #2: The facility infection control policy, provided to the Licensing Inspector on 12/20/2021, under "Hand Washing" reads, "Residents are asked to wash hands or use waterless antiseptic before and after restroom use and before eating meals to avoid infection."

Standard #: 22VAC40-73-750-E
Description: Based on observation of the facility physical plant and resident interview the facility failed to ensure the facility shall have sufficient bed linens in good repair so that residents have clean sheets. Evidence by photos taken.

Evidence #1: While observing the second floor bedrooms, two Licensing Inspectors observed 4 out of 4 resident beds that did not contain sheets.

The Licensing Inspector interviewed resident #1 and the resident informed that bed sheets are not provided to the resident. The Licensing Inspector asked if the resident would like bed sheets and the resident responded yes.

Evidence #2: While interviewing staff #2, the staff stated that resident #1 removes the sheets off of the bed.

Standard #: 22VAC40-73-860-I
Description: Based on observation of the facility physical plant, the facility failed to ensure hazardous materials are in a locked area. Evidenced by photos taken.

Evidence: On 12/7/2021 with staff #1, the Licensing Inspector observed garden tools and numerous pieces of l broken plywood in the facility back yard and side upper deck accessible to the facility residents. On the side upper deck there was a makeshift table out of plywood and milk crates.

Description: Based on observation of the facility physical plant, the facility failed to ensure the interior and exterior of the building shall be maintained in good repair. Evidenced by photos taken

Evidence#1: With staff #1, the Licensing inspector observed the following in the rear of the facilities yard:

1-Milk crates;
2-Beverage containers and boxes;
3- Vinyl siding;
4- Carpet fabric;
5- Snack wrapper's;
6- Multiple buckets;
7- One bicycle wheel;
8- One metal ladder;
9-Large push barrel containing bricks;

Evidence #2: While observing the facility interior with staff #1 and #2, the Licensing Inspector observed the following area not in good repair.
1-The first floor ceiling contained brown stains;
2- The first floor dining area had a broken glass window;
3- The brown wall in the first floor dining room was peeling;
4- The second floor restroom was discolored along the baseboard;
5- The facility first floor was missing flooring in multiple area located between the facility dining area leading into laundry area;
6. The first floor dining area contained plywood boarding over window;
7. The second floor bathroom wall was peeling;
8, The second floor hallway contained blankets, syrofoam cups, and a large metal unit;
9. The stair rail leading to the facility second floor was not secured;

Description: Based on observation of the facility physical plant, the facility failed to ensure the grounds shall be kept free of breeding places. Evidenced by photos taken

Evidence: On 12//07/2021 while observing the facility physical plant with staff #1 and #2, the Licensing Inspector observed a sitting, yellow mop bucket which contained:

1- A brown liquid;
2- Toilet tissue roll in the brown liquid;
3- Flying gnats inside the mop bucket;

Standard #: 22VAC40-73-870-E
Description: Based on observation of the facility physical plant, the facility failed to ensure all furnishings including bathtubs and showers shall be kept clean and in good repair and condition. Evidenced by photos taken.

Evidence #1: On 012/07/2021 with staff #1 and #2, the Licensing Inspector observed the 2nd floor walk in shower was discolored black, blue and green, in addition to missing tiles on the shower floor. The bathroom sink was brown between the rear sink and wall. The bathroom toilet lid was removed from the toilet and located in the floor.

Evidence #2: The second bathroom located on the same floor hand washing sink was located in the facility bathtub.

Description: Based on observation of the facility physical plant, the facility failed to ensure inside stairways shall have nonslip surfaces. Evidenced by photos taken.

Evidence: On 12/07/2021 with staff #1 and #2, the Licensing Inspector observed the interior facility stairs leading to the second floor tread was missing on multiple steps, worn and broken on additional steps.

Evidence : On 12/07//2021 with staff #1, the Licensing Inspector observed areas accessed by the facility residents that contained overgrown weeds.
