Comments:

A non-mandated monitoring inspection was initiated on 01/04/2022 and concluded on 01/04/2022. A self-report from the facility was received by the department regarding the area of personnel. The Licensing Inspection spoke with the Community Director regarding the self-report via phone. The evidence gathered during the non-mandated monitoring inspection supported the self-report of non-compliance with standards or law, and violations were issued. Any violations not related to the self-report but identified during the course of the investigation can be found on the violation notice.

Standard #:	22VAC40-73-150-A
Description:	Based on staff email and interview, the facility failed to ensure there is an administrator of record. EVIDENCE: 1. In an email from staff 1 on 12/17/2021 at 2:57 PM, staff 1 stated that the administrator of record, staff 2, "walked off the job on Tuesday (12/14/2021) and has not returned.". 2. Phone interview with staff 1 on 01/04/2022 confirmed that staff 2 is no longer employed at the facility and that there has been no administrator of record at the facility since 12/14/2021.
Plan of Correction:	Facility did not fail to ensure compliance. Staff 2 gave notice on 12/2/2021 to work until 2/3/2022 or until a replacement was hired and trained. Staff 2 abruptly "walked off job" on 12/14/2021 and has not returned. 1. Staff 1 is actively looking for a qualified replacement for staff 2 "Administrator of Record" or for a possible preceptor to provide training to a current employee. 2. Rising Covid concerns and staffing shortages have greatly contributed to a lack of qualified canidates.
