Comments:
An unannounced renewal study was conducted from 7:45 a.m. - 3:00 p.m. on 10/28/2021. At the time of entrance 68 residents were in care. The sample size consisted of four resident records and four staff records. Residents and staff were interviewed. Resident and staff records and other documentation reviewed. Criminal Background Checks of all staff hired since previous inspection conducted on 04/04/2020 were reviewed. Residents were observed eating breakfast and lunch and engaging in activities including exercise in safe secure area and lecture and learn in assisted living area. Beauty parlor services was observed and PT assistance in hall was observed. Medication administration was observed with three staff. Three medication carts observed for PRN medications. Building and Grounds observed. Additional documentation requested to be emailed.

Violation notice issued and risk ratings reviewed with administrator at exit interview on 12/07/2021.

Areas of non-compliance are identified on the violation notice. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return to the licensing office within 10 calendar days.

Please specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. The plan of correction must contain: 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the non-compliance from occurring again, 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s), and 4) date that that plan of correction will be completed.

Description: Based on record review, facility failed to ensure that the facility shall implement a written plan for medication management with methods to prevent the use of outdated medications.

EVIDENCE: The medication cart during observation of administration included Resident 3's routine Novolog that expired 8/2021 and Resident 1's PRN Refresh that expired 3/2021; both medications were replaced with current dates prior to administration.

Description: Based on record review, facility failed to ensure that the criminal history record report shall be obtained on or prior to the 30th day of employment for each employee.

EVIDENCE:
Staff 14 was hired on 11/16/2020 with a Criminal History Record dated 5/12/2021.
