Comments:
One licensing inspector conducted an inspection with the local department regarding a complaint received by the licensing office involving the physical plant at Fort Shelby Manor on 01/05/2022. The complaint was received on 01/03/2022. The inspection started at 10:40 am and concluded at 11:30 am. During the inspection resident and staff interviews were conducted, the physical plant was observed and documentation was reviewed. An exit meeting was held with the administrator on 01/05/2022 and at that time opportunity was given to find items not available during the inspection. As a result of this inspection the complaint was determined to be valid and 1 violation is being cited. Please complete a plan of correction for each violation cited along with a date of correction. Please return a signed and dated copy back to the licensing office within 10 calendar day (01/23/2022) of receipt. If you have any questions or concerns please contact your inspector at 276-608-3514. Description: Based on interviews with residents, staff, and observations made during the inspection of the physical plant, the facility failed to ensure buildings shall be kept free of infestations of insects and vermin.

EVIDENCE:
1. The Licensing inspector received information from the local department regarding a complaint of bedbugs within the facility. The complaint report was received by the Department on 1/3/2022.
2. According to staff # 1 the facility has been dealing with bedbugs for a few months. She stated "we have treated every individual that we can with prescription cream that was ordered by the physician." Staff # 1 advised that resident # 1 had the worst rash as a result of the bedbugs.
3. Upon inspection of resident rooms the licensing inspector observed residue on the curtain in the room located off of the dining room area that appeared to be bedbug residue. Staff # 1 confirmed that it was residue left by bedbugs.
4. The Licensing Inspector interviewed resident # 1 regarding issues with bedbugs. The licensing inspector observed several bites and scratch marks on this resident's torso. He stated "the rash is very itchy and they put cream on me to help." His room is located off of the dining room area. He denied ever seeing bedbugs in his room and they wash his sheets every other day.
5. Staff # 2 also confirmed there has been an ongoing issue with bedbugs. She advised they are being treated by an outside source and they are also using bedbug spray and powder to resolve the issue. She also advised that some resident's are noncompliant with treatment.

Plan of Correction: Staff will check ever bed daily for any signs of bedbugs. All bedding is washed on a weekly schedule; however, if bedbugs are found the bedding is washed then. The bed is sprayed including the mattress, box springs, bedframe. chests and curtains. Also, powder is put around the feet of the bed. Also, the residents clothes are washed. We use the spray or powder recommended by Virginia Tech and the process that they recommend. We will strive to encourage all resident to cooperate with our process to eradicate the problem altogether. Also, any time a new resident is admitted all their clothes are washed. [sic]
