Comments:
A non-mandated complaint inspection was initiated on 10/20/2021. A complaint was received by the department regarding allegations in the areas of resident care and related services and building and grounds. The licensing inspector conducted an on-site observation at the facility on 10/20/2021 and further documentation was requested to be emailed to the LI for review. The evidence gathered during the investigation supported the allegation of non-compliance with standards or law, and violations identified during the course of the investigation can be found on the violation notice.

Description: Based on observations made during an on-site inspection conducted on 10/20/2021 and review of the facility posted activity calendar, the facility failed to provide activities posted on the activity calendar.

EVIDENCE:

1. The facility posted activity calendar for the AL unit was noted to have documentation on 10/20/2021 for daily devotionals at 10:30am and Bible Stories with Eversound at 11:00am. It was observed by both LI's that these activities did not occur at the times documented. Interview with staff indicated that the activities did not take place.

2. The facility posted activity calendar for the memory care unit was noted to have documentation on 10/20/2021 for daily devotionals at 10:30am and Wednesday Wisecracks at 11:00am. It was observed by both LI's that these activities did not occur at the times documented. Interviews with staff indicated that the activities did not place.

Plan of Correction: Adequate program staff has been implemented and will provide activities to residents in both AL and MC per DSS regulations. Programing director will create monthly calendar of activities for all residents within the community. Programing staff will facilitate programing as indicated by the daily calendar. Program Director will conduct and monitor activities

Description: Based on observations made of the facility physical plant, the facility failed to ensure that the current weeks menu was posted in a conspicuous area to all residents.

EVIDENCE:

1. Weekly menus for July and August 2021 were observed to still be posted in the facility on the day of inspection.

Plan of Correction: Menus have been printed and posted Monthly, and daily menus will be printed and posted in accordance to DSS regulations. Dining Director will ensure menus are posted timely.

Description: Based on observations made of the facility physical plant, the facility failed to keep the interior of the building clean.

EVIDENCE:

1. Rooms 511 and 514 were noted to has trash/debris on the floor.

2. Carpet stains were noted in the hallway on the memory care unit.

Plan of Correction: Adequate housekeeping staff has been hired. Housekeeping services will be performed for each resident and in common areas daily.
Director of Housekeeping will monitor daily housekeeping activities.
