22VAC40-73 RESIDENT CARE AND RELATED SERVICES

Comments:
One licensing inspector conducted a focused monitoring visit regard a self reported incident at Commonwealth Senior Living-Radford on 1/13/2022, this inspection was concluded 2/4/2022. The monitoring inspection began at 12:00 and concluded at 12:30pm/ The licensing inspector reviewed all necessary documents from the facility and the physician. An exit meeting was held with the administrator on 2/7/22. Two violations are being cited as a result of this monitoring inspection. Violations:
Standard #: 22VAC40-73-320-A
Description: Based on observations made during the review of the resident record, the facility failed to ensure that within 30 days prior to admission a physical exam was completed and included all required information.
EVIDNECE:
1. Resident #1 was admitted to the facility on 12/29/2021.
2. Resident #1 had a physical exam completed by a physician on 12/28/2021 which did not address the ability or lack thereof to self-administer medication.
3. Resident #1 had Aspirin documented as an allergy on the physician?s note attached to the physical dated 12/28/2021, there was no reaction documented for this allergy.

Plan of Correction: Resident discharged prior to violation. Thorough medication audit and review by RCD, ED, house PCP and partnering pharmacy.
ED and RCD to inspect History & Physical thoroughly before approving admission. [sic]

Standard #: 22VAC40-73-680-D
Description: Based on observations made during the review of the resident record, the facility failed to administer medications in accordance with the physician?s or other prescriber?s instructions and consistent with the standard of practice outlined in the current registered medication aide curriculum approved by the Virginia Board of Nursing.
EVIDENCE:
1. Resident #1 was admitted to the facility on 12/29/2021.
2. Resident #1 had a physical exam completed and signed on 12/28/2021. This physical exam report included current medications which was signed by the physician which included: Pravastatin 20 mg one tablet by mouth daily; Probiotic 20 billion cell take one capsule daily; Melatonin 5mg take one tablet at bedtime; Sotalol 80mg one tablet twice daily; Vitamin D3 125mcg take on tablet by mouth daily; Co Q-10 200mg take two capsules by mouth daily; Omeprazole 20mg delayed release take one capsule by mouth daily; Valsartan 80mg take one tablet daily; Rytary 23.75mg-95mg extended release take one capsule twice daily; Xarelto 20mg take one tablet by mouth daily; Montelukast 10mg take one tablet daily; Budesonide-fomoterol HFA 160mgc-4.5mcg/actuation aerosol inhaler, inhale twice daily.
3. The facility did not send this list to the pharmacy to be filled. Resident #1 was not administered any of the above mentioned 12 medications from 12/29/2021-01/05/2022.

Plan of Correction: ED and RCD has done a full medical chart audit on every resident in the facility to ensure accuracy and compliance.
ED and RCD to inspect H&P thoroughly before admission. Entire History & Physical to be sent to pharmacy for accurate MAR documentation. [sic]
