Comments:

An unannounced monitoring inspection was completed from 11:00 a.m.-12:00p.m. by Licensing staff. There are 8 residents in care at this time and 8 were on the premises during the inspection. 2 staff members on duty. Follow up was completed on the previous violation notice and all violations were corrected. A tour of the physical plant was completed. A sample size of 4 resident's records and 3 staff records were reviewed for compliance. No new staff hired since last inspection. Residents were engaged in guitarist on site musical appreciation activities during the inspection while waiting for lunch to be served. Physician orders, medications administration records observed. Standard #:	22VAC40-73-320-B
Description:	Based on a review of resident's records, all records did not have documentation of current annual tuberculosis testing or screening. Evidence: Three of four resident's records reviewed did not have current TB screenings or tests. Resident's #1 last screening date was 5/4/2019, resident's #3 last screening date was 4/5/2019 and resident's #4 last screening date was 6/3/2020.
Plan of Correction:	Administrator and designated staff will schedule for all residents to have the TB tests and or screening completed on the designated form and place in their records. A form will be developed to keep track of annual dates for screenings to be in compliance in the future.
Standard #:	22VAC40-73-550-G
Description:	Based on a review of resident's records, four records did not have documentation of annual review of resident's rights and responsibilities. Evidence: Resident's #1 last review is dated 4/4/2020, resident's #2 review is dated 11/4/2020, resident's #3 review is dated 4/4/2019 and resident's #4 review is dated 6/13/2020.
Plan of Correction:	Administrator and designated staff member will make sure to review and document the resident's rights and responsibilities with all residents and their responsible parties. All will sign and date the form and it will be placed in each resident's record. A form will be implemented for all annual reviews to be in compliance with the standards.
