Comments:
An unannounced complaint inspection was conducted in response to complaints received by the licensing office on 12/27/21 and 12/29/21. Building and grounds were inspected and records were observed. Violations were discussed and an exit meeting was held. Complaint regarding Building and Grounds was deemed valid, as a preponderance of evidence gathered during the investigation supported the allegation. Areas of non-compliance are identified on the violation notice. Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return to the licensing office within 10 calendar days. Please specify how the deficient practice will be or has been corrected. Just writing the word 'corrected' is not acceptable. The 'plan of correction' must contain: 1) Steps to correct the non-compliance with the standards, 2) Measures to prevent the non-compliance from occurring again, and 3) Person responsible for implementing each step and/or monitoring any preventative measures. Description: Based on observation, the facility failed to ensure that a medicine cabinet, container, or compartment is used for the storage of medications and dietary supplements prescribed for residents when such medications and dietary supplements are administered by the facility. Medications shall be stored in a manner consistent with current standards of practice.
Evidence: Building and grounds were inspected, during the inspection. A yellow pill, with a "100" imprint, was found in the common area of the Dockside neighborhood.

Plan of Correction: RSC will provide re-training to LPNs and Medication Techs on current standards of medication storage and administering medications to residents.

RSC and/or designee will include inspection of common areas for dropped medications, to their medications and MARs weekly audits.

RSC and/or designee will provide training and/or retraining to LPN's and MT's on medication storage and administration of prescribed medications. This training will be conducted and reviewed by the RSC and/or designee on orientation, annually and, as needed.

RSC, Executive Director or designee will review the weekly audits.

Description: Based on observation, the facility failed to ensure that the building is well-ventilated and free from foul, stale, and musty odors.
Evidence: Building and grounds were inspected, during the inspection. A urine odor was present on the Dockside neighborhood. Resident rooms were checked and dried liquid was observed on Resident #1's bathroom floor.

Plan of Correction: Resident #1's bathroom floor was cleaned immediately. The exhaust belt in the exhaust motor was replaced and therefore ventilation improved. Caregivers will attend an in-service on light housekeeping and expectations.

The management team will inspect resident rooms to ensure compliance.

Housekeeping will continue to monitor cleanliness of bathrooms daily.

RSC and/or designee will conduct random audit room rounds to ensure compliance.
