Comments:

Two licensing inspectors conducted a one day unannounced mandated licensing monitor inspection at Gregory?s Rest Homes on 03/01/2022. The inspection began at 10:50 am and concluded at 1:26 pm. During this inspection required postings were checked, the noon medication pass was observed, Medication cart audits were conducted, medication administration records were reviewed, lunch was observed, buildings were observed, resident and staff interactions were observed, and a sample of resident and staff files were reviewed. There were 24 residents in care at the time of the inspection. An exit meeting was conducted with the administrator on 03/01/2022 and at that time opportunity was given to find items that were not available in files. As a result of this inspection 1 violation is being cited. Please develop a plan of correction for each of the cited violations along with a date of correction and return a signed and dated copy back to the licensing office within 10 calendar days (03/14/2022) of receipt. Description:	Based on observations made during the noon medication pass, the facility failed to administer medication in accordance with the physician's order for one resident. EVIDENCE: 1. Resident # 6 is prescribed Diclofenac Sodium 1% gel to be applied topically to bilateral knees and left great toe three times a day for arthritic pain. 2. Staff # 2 did not administer the noon dose of gel to the knees nor to the left great toe; although she initialed the MAR as if she had administered the medication. 3. Staff # 2 stated that she has not been applying the gel to the left great toe area for this Medication Administration Record (MAR) period which is 2/19/2022-03/20/2022. 4. Staff # 2 initialed the MAR indicating she had administered the gel according to physician's orders for the morning and noon dose on the date of the inspection; 3/1/2022. 5. The Diclofenac Sodium 1% gel was not on the medication cart, but kept in the bedside table in Resident # 6?s room. There was no physician's order available to indicate that Resident # 6 could keep the gel in her bedside table.
