Comments:
Two licensing inspectors conducted an unannounced non-mandated focused monitoring inspection at Carrington Place in Wytheville on 02/28/2022. The inspection started at 10:35 am and concluded at 12:00 pm. The purpose of this inspection was to look at previous violations and determine compliance with standards using the most recent inspection protocol. During the inspection the building was observed, resident and staff files were reviewed, medication carts were observed, physicians orders and medication administration records were reviewed. An exit meeting was held with the director of nursing and other key staff on 02/28/2022 and at that time opportunity was given to find items not available during the inspection. As a result of this inspection one violation is being cited. Please develop a plan of correction for the violation cited along with a date of correction and return a signed and dated copy back to the licensing office within 10 calendar days (03/17/2022) of receipt. Violations:
Standard #: 22VAC40-73-640-A
Description: Based on observations made during the audit of the medication carts, the facility failed to adhere to their medication management plan regarding methods to prevent the use of outdated, damaged or contaminated medications and to ensure that each resident's prescribed medications and any over the counter drugs or supplements ordered for the resident are filled and refilled in a timely manner to avoid missed doses.

EVIDENCE:
1. Resident #1 is prescribed Adrain Disk,one puff twice daily and rinse mouth. This medication was observed to have no open date.
2. Resident # 2 is prescribed Lantaprost Sol 0.005%, apply one drop to each eye, Econazole Cream 1%, apply small amount to toenails every day, Enulose 30 MLS every day, none of these medications had an open date documented. Resident # 2 is also prescribed Pataday Solution 2%, apply one drop in each eye daily at bedtime. There was no open date on this medication, the bottle was observed to be empty and the medication could not be located within the facility.
