Description:	Based on observations made during the audit of the medication carts, the facility failed to adhere to their medication management plan regarding methods to prevent the use of outdated, damaged or contaminated medications and to ensure that each resident's prescribed medications and any over the counter drugs or supplements ordered for the resident are filled and refilled in a timely manner to avoid missed doses. EVIDENCE: 1. Resident #1 is prescribed Adrain Disk,one puff twice daily and rinse mouth. This medication was observed to have no open date. 2. Resident # 2 is prescribed Lantaprost Sol 0.005%, apply one drop to each eye, Econazole Cream 1%, apply small amount to toenails every day, Enulose 30 MLS every day, none of these medications had an open date documented. Resident # 2 is also prescribed Pataday Solution 2%, apply one drop in each eye daily at bedtime. There was no open date on this medication, the bottle was observed to be empty and the medication could not be located within the facility.
