Comments:

The licensing inspector for Commonwealth Senior Living at Radford conducted a one day unannounced complaint inspection beginning at 9:45am and ending at 10:15am on 03/31/2022. The allegations being reviewed were resident care and physical plant. Based on a review of evidence, the complaint was determined to be not valid. No violations are being cited from this complaint investigation. An exit interview was held with the administrator of the facility 03/31/2022, during this time the opportunity it locate items that were needed was given.
