An unannounced onsite complaint investigation was initiated on 03/22/2022 and concluded on
03/24/2022. The allegations made against the facility were in regards to resident care, services and
supervision. The facility offered 81 residents in care. During the investigation the inspector reviewed facility records, and conducted interview with residents and staff.
During the exit interview the inspector offered a review of the allegations and technical assistance
regarding resident care, services and supervision.
This investigation determined there were no violations with applicable standards or law. No violations were issued.
