Comments:

On 3/22/2022 a complaint investigation was begun, and was concluded on 4/25/2022. Resident records, a staff record, and other documents were reviewed. Staff and resident interviews were done. One of the alleged incidents of non-compliance had a preponderance to support it, so the complaint is valid. An onsite exit interview was done on 3/22/2022 and the facility was given an opportunity to provide additional evidence. An exit interview was held by phone on 4/28/2022.

Description:	Based on staff interview and documentation, the facility failed to allow a resident to exercise Resident Rights and Responsibilities (Section 63.2-1808. A.10) - "Is free from ... physical ... abuse... ." EVIDENCE: 1. On 3/8/2022, the facility reported that staff 1 allegedly slapped resident 1 while attempting to provide care. This was substantiated by three eyewitnesses.
Plan of Correction:	Staff 1 was terminated from employment on 3/9/2022, after the facility completed an internal investigation for what other staff reported to management . Remaining staff have been given additional training on abuse, timely reporting of alleged abuse, and mandated reporter requirements.
