Comments:

On 3/22/2022 one inspector began a complaint investigation (11:35 am to 4:23 pm). Staffing schedules were reviewed, staff were interviewed, and there is no evidence to support the allegation of non-compliance. An exit interview was held on site the day of the inspection and the facility was given an opportunity to submit additional evidence. A telephone exit interview was held on 4/28/2022.
