An unannounced monitoring inspection was conducted at the facility on 04/20/2022 between the approximate time of 11:28a.m and concluded at approximately 2:10p.m. During the entrance interview the facility Administrator offered sixty-nine residents in care. Based on the census offered the inspector reviewed four resident records, four staff records and other facility documentation for compliance. The inspector also observed a midday medication administration pass, lunch time meal conducted a walk-through of the facility and conducted interviews with residents and staff.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.
An exit interview was conducted where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.

Description:

Plan of Correction: FACILITY'S RESPONSE "Employee #5 received written counseling regarding the importance of remaining in compliance with CDC and OSHA guidelines on 4/22/2022. Employee #5 also received verbal counseling from Executive Director on the importance of following appropriate PPE guidelines at all times.
Executive director will continue to provide the community with education, training and frequent rounding to ensure that the community remains within compliance at all times"
