Description:	Based upon documentation and an interview, the facility failed to ensure four of the six July 2022 medication administration records (MARs) reviewed included all required information. Evidence: 1. Clonidine for resident 2 did not have a diagnosis. 2. Loxapine for resident 3 did not have a diagnosis. 3. Aripiprazole, Flonase, Quanfacine and Sertraline for resident 5 did not have the diagnoses. 4. Clozapine for resident 6 did not have a diagnosis. 5. On 7/22/2022, the licensing inspector (LI) interviewed the administrator who stated the diagnoses were on the physicians? orders; however, they were not on the MARs.
Plan of Correction:	Diagnosis was added for each medication to individual MARs by RMA before LI left the building on 7/22/2022. Administrator and administrator?s assistant will be responsible for checking the MARs each month to ensure information is accurate and complete. Physician and or pharmacy will be contacted if any information needs clarification. When necessary, administrator/administrator's assistant will write missing information found on the physician?s order sheet onto the MAR. Copy of MAR regulations will be printed and placed by the medication cart so all RMAs will know exactly what is required and have a reminder at their fingertips.
