Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 06/28/2022 start: 9:10am-1;25pm The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. A complaint was received by VDSS Division of Licensing on 6/3/22; 6/17/22; 6/23/22; 6/24/22; 6/28/22 regarding allegations in the area(s) of: neglect, elopements, not able to meet resident needs; sexual assault resident to resident, staff not treating family with respect, residents not being treated with dignity, medication errors leading to overdose. The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. The evidence gathered during the investigation did not support the allegations made in the complaint of non-compliance with standard(s) or law.
